When the pain is in your knee but the problem started at your pelvis. A whole-body assessment that finds what treating the sore spot alone missed.

You have treated the painful spot directly, more than once, and the relief never quite sticks. That pattern often means the actual source of the problem is somewhere else in the chain entirely. The Hesch Method is a manual assessment and treatment approach, developed by physical therapist Jerry Hesch, built on the idea that the body works as one connected biomechanical system rather than a set of separate parts. Instead of examining only the area where you feel pain, the therapist evaluates the relationships between your spine, pelvis, and extremities, looking for subtle joint restrictions or asymmetries that may be quietly driving symptoms somewhere down the chain. A slight rotation or restriction at the pelvis, for example, can alter how load travels through the hip, knee, or even the ankle, showing up as pain in a joint that itself is not actually the problem. The assessment uses precise, passive movement testing to detect these small mechanical asymmetries that a general screening often misses, then applies targeted manual correction to restore proper joint alignment and movement mechanics before addressing the symptomatic area directly.
This approach depends heavily on the therapist's hands. Detecting a subtle restriction at the pelvis or a small asymmetry between two joints takes advanced training beyond a standard physical therapy program, along with a refined sense of touch built through experience. Because of that, the Hesch Method is typically used alongside other manual therapy and corrective exercise, giving your body both the mechanical correction and the strength to hold that correction once it is made.
The Hesch Method is for pain that has not fully made sense to you or a previous provider. If treating the obvious spot has not held, the answer may be somewhere else in the chain.
You have iced it, stretched it, even had hands-on treatment directly on the area, and it keeps coming back. A whole-body assessment looks upstream and downstream of the pain to find what is actually driving it, rather than repeating the same local treatment.
Pain that radiates from your low back or pelvis into your hip, thigh, or leg often has a mechanical origin at the pelvis itself. This method is built specifically to detect and correct that kind of subtle asymmetry at its source.
Years of favoring one side or one movement pattern can leave several areas compensating for the original problem. A careful, whole-body assessment untangles which restriction came first and treats that root cause instead of the compensations built on top of it.
A Hesch Method session is deliberate and detailed, moving from a full-body evaluation to precise, targeted correction of what the evaluation finds.
Your therapist assesses the relationships between your spine, pelvis, and extremities, testing passive movement to find subtle restrictions or asymmetries, including ones in areas you have never associated with your pain at all.
Using gentle, specific manual techniques, your therapist targets the exact restriction identified in the evaluation. This is careful, focused work, not general massage, aimed at restoring proper joint mechanics at the source of the problem.
Your therapist retests the movement pattern to confirm the correction held, then prescribes targeted exercise to reinforce it, so your body learns to maintain the new alignment on its own between visits and over time.
Tell your therapist about any recent fractures, surgeries, or areas of acute injury before your assessment, so technique and pressure can be adapted around them safely. It is also common to feel sensation in an area away from your primary pain, that is simply a sign of how connected the correction really is.
